AVHS Public Veterinary Medical Assistance Fund is to provide ONE TIME financial assistance not to exceed $500 to low income residents of the City of Auburn who use a Washington State licensed veterinarian from the AVHS VetPartners list for non-routine medical care of their dogs and cats.
The goal is to enrich and maintain the human animal bond and keep loved pets in their homes and to reduce the possibility of unwarranted euthanasia due to the inability of pet owners to fund needed costs for medical care.
The AVHS Public Veterinary Medical Assistance Fund provides reimbursement for non-routine medical care up to $500 performed by an AVHS VetPartners veterinarian within the previous week or direct veterinary payment assistance for medical care provided by an AVHS VetPartners veterinarian as available per program funding and donations to the fund. Proof of low-income status and Auburn residency is required before authorizing medical services to AVHS VetPartners. Please view list of VetPartners here.
Please note*** Funding through this program is not available for routine office visits, vaccines, routine diagnostics or routine spay/neutering services.
The AVHS Public Veterinary Medical Assistance Fund is limited to ONE pet per household and your lifetime.
Once you use the AVHS Public Veterinary Medical Assistance Fund, you will NOT be able to receive assistance again.
This fund is a onetime use only.
The AVHS Public Veterinary Medical Assistance Fund will give preference to animals that are spayed or neutered and licensed in the City of Auburn.
For consideration of financial assistance for veterinary medical costs for your pet through the AVHS Public Veterinary Medical Assistance Fund, please complete and return the Public Veterinary Assistance Fund Application to AVHS.
Additionally, all applicants must present in person, mail, or email:
proof of low income status,
the veterinary medical estimate(s),
invoices or receipts, and
proof of residence in the City of Auburn to AVHS within 5 days of sending your initial request.
Once we receive a request for the AVHS Public Veterinary Medical Assistance Fund, it is our goal to contact you within 5 business days after the receipt of supporting documentation to initiate the process. If you fail to submit supporting documentation within 5 days, your application will be declined.
